Here at the Aesthetic Center, we offer various cosmetic surgery techniques and procedures that are designed to enhance your appearance and self-confidence. We are located in Bismarck, North Dakota, but we invite patients from surrounding areas such as Fargo and Minot to come in and discover how our procedures can change their appearance – and their life.

Dr. Sara Fiene, M.D.

Sara Fiene, MD, FAACS is North Dakota’s only board-certified cosmetic surgeon, dedicated to helping patients feel confident and empowered through safe, artfully performed aesthetic procedures. Born in Wichita, Kansas, but raised in Bismarck, North Dakota, Dr. Fiene considers North Dakota home. She earned her Bachelor of Science in Biology, graduating Summa Cum Laude, with a minor in Spanish from the University of North Dakota in 2015, followed by her medical degree (MD) from the UND School of Medicine in 2019.

She completed her residency training in Wichita, Kansas, at the very facility where she was born a full-circle moment, as she graduated on her 30th birthday. During residency, Dr. Fiene developed a strong foundation in women’s health and surgery, which inspired her pursuit of further specialization in cosmetic surgery.

Drawn to the blend of surgical precision and creative artistry, Dr. Fiene was selected for and completed an intensive, high-volume fellowship in cosmetic surgery at a renowned practice in Salt Lake City, Utah. This hands-on training equipped her with advanced skills in a full range of aesthetic procedures for both women and men.

Known for her warm, down-to-earth approach, Dr. Fiene believes in personalized care and natural-looking results that enhance each patient’s individual beauty and youthfulness.

Outside of her surgical practice, she enjoys spending quality time with her family and her beloved cat, Sophie, and dog, Tina. She and her husband love to attend local events that support the beautiful community she grew up in; the same community that supports her practice. She’s an avid cook, always up for trying new recipes or exploring local restaurants and craft breweries.

Dr. Rick Paulson, M.D.

In January 2025, after more than 20 years of dedicated service to the Bismarck, Minot, and Dickinson communities, Dr. Paulson announced his retirement.

Throughout his esteemed career, Dr. Paulson distinguished himself by combining surgical precision with an unwavering commitment to patient care. Under his leadership, the Aesthetic Center grew to include a fully accredited surgery facility and became a trusted destination for those seeking advanced, personalized treatments.

We extend our sincere gratitude to Dr. Paulson for the skill, professionalism, and integrity that defined his practice and shaped the Aesthetic Center. His legacy will continue to guide and inspire our team as we carry forward the same commitment to excellence and patient‑focused care that he exemplified throughout his career.

We are also pleased to announce that Dr. Sara Fiene, who joined the practice in 2024, is excited to carry this legacy forward, continuing to serve our patients with the same dedication, compassion, and expertise that have long been the hallmark of the Aesthetic Center.

Kara, RN, BSN

Bismarck / Minot

Kara is a registered nurse in the Bismarck and Minot clinics. She graduated from Medcenter One College of Nursing in 2010 and has been a part of the team at ACPS since 2016. She currently circulates in the OR and is trained in all laser procedures and Morpheus8. Kara also functions as a nurse injector of neurotoxins (her favorite!), dermal fillers, and sclerotherapy. Her advanced training in our medical-grade skincare line ZO Skin Health helped her receive her ZO Expert Certification, doing skin consultations and performing aesthetic skin treatments, such as Hydrafacials, microneedling, dermaplanes, and chemical peels. Outside of work, she enjoys reading, napping, and spending time outside with her husband and 2 sons.

Kelah, RN, BSN

Bismarck

Kelah graduated with her BSN from North Dakota State University and started her career in the Emergency Department. She started working with Dr. Paulson in February 2023, where she helps in the OR with pre-op and recovery. On non-surgery days, she helps on the clinic side and enjoys doing laser hair removal treatments.

She loves working at the Aesthetic Center because her passion for helping others feel their best meshes with her other job as a hairstylist. She loves seeing patients from the first time they walk in for a consultation to 3 months post-surgery. It’s so rewarding to see how far they have come and their confidence boost! Kelah’s favorite treatment to receive would be toxin injections – she loves how refreshed she looks for months after.

She enjoys working out, spending time with family, and watching all the sports in her free time!

Leann, RN, BSN

Bismarck

Leann graduated with her BSN from North Dakota State University and has been employed as a registered nurse for over 14 years. After working as a travel nurse throughout the US, she returned to ND in 2014 to be near family. Leann started with Dr. Paulson in March 2015. She is a primary clinic nurse in Minot and Bismarck and is part of our OR team working in preop and recovery. In 2016, she took a particular interest in bioidentical HRT and is currently the coordinator and educator for our HRT patients.

Leann is one of our aesthetic treatment nurses in Bismarck, She has performed injections since 2015 taking care of patients’ aesthetic needs with neurotoxins, dermal fillers, and biostimulators. She also has specialized training in many procedures, including hair removal, tattoo removal, skin resurfacing, vascular treatments, and miraDry. Morpheus8 and Pixel8 are two of her favorite treatments to perform!

She has no free time as she has two young children and enjoys sleep. Eventually, she’ll travel and read a non-picture book.

Tina, RN, BSN

Dickinson

Tina travels to Dickinson to provide aesthetic injection services in our outreach clinic. She graduated from Medcenter One College of Nursing and brings 20+ years of nursing experience to the practice. Tina started at the Aesthetic Center in 2012. She has extensive experience using Botox, Dysport, Jeuveau, Restylane Dermal Fillers, Versa, and Sculptra Aesthetic Injectables. She has received training from Allergan, Galderma, and Revanesse nationally recognized trainers, attending conferences to keep current on the latest trends and best practices.

When Tina is not working, she loves to spend time on Lake Sakakawea in the summer and stays cozy in her home in Beulah in the winter, reading books or woodworking.

Tina’s favorite injectable treatments are Dysport, Sculptra, and Restylane Kysse. She is passionate about helping women slow down the aging process and giving them a younger, refreshed look.

Tina loves all ZO products. She feels that taking care of your skin and being on a regimen of ZO skincare is the most crucial aspect of skin health.

Kallie, FNP-C

Minot

Kallie RN works in the Minot clinic. She is from Minot and graduated from Medcenter One College of Nursing in 2010. She brings 5 years of nursing experience to the practice. Kallie has experience in injecting neuromodulators, dermal fillers, and Sculptra. She has received training from nationally recognized trainers out of MN, along with training from Allergan and Galderma expert injectors. She has skincare training from ZO, Obagi, and is PCA certified. She also performs laser hair removal, sclerotherapy, laser skin rejuvenation, and acne treatments, as well as medical-grade chemical peels and medical microdermabrasion. Kallie started with Dr. Paulson in January 2015.
